A side stitch is a sharp pain on right or left side just under the diaphragm that usually occurs during exercise. Most people experience side stitches when they are new to exercise. As you get in better shape and your body adapts to the stress of running, lifting, jumping, etc. you will stop getting these annoying pains when working out. Side stitches are not dangerous, however they can be pretty painful and interrupt your workout. The best thing to do if you experience a side stitch is to stand-up tall, pull your arms over your head, and keep walking around. Most importantly, focus on taking slow deep breaths in through your nose and out through your mouth. Most people hunch over or sit down when they have a stitch which only makes the pain last longer. To prevent stitches from every occurring, focus on keeping a rhythmic breathing pattern. Stitches can be linked to erratic and uncontrolled breathing.

Two days ago I made a house call (yes, Dr. Posner still does that occasionally…a true Dinosaur…middle age primary care Jewish doctor who makes house calls) to a very unfortunate medical patient who has metastatic, terminal cancer. He is an immigrant from South America, family takes great care of him, and has no insurance. Capital Hospice, a great organization, provides nursing care. Pain control and allowing him to die in dignity are the main goals. He is “alive” but is this really “life”? QUALITY of life is much more important than QUANTITY of life. God forbid, if any of us were in this situation, effectively all of the things that are/have been important to us are taken away. This is what a catastrophic medical condition can bring. Unfortunately, as we are all immersed in our daily work tasks, family responsibilities etc, we do not think about these issues as for many, a human nature tendency is to think “this could not happen to me..this happen to other people.” Think again. If you have numerous risk factors for a severe medical issue, then the chances that something really bad will happen to you are very much increased. Please take the one second pause to think about this if you have a weight control issue and that “sweet” appears in front of you. In the aggregate, are these derailing food sources offering immediate gratification really worth it?

Many people have asked me if there is a correlation between sweating a lot and being out of shape. When you’re moving your body, you generate heat. The body produces sweat which then evaporates on the skin to help you cool off. The rate at which we sweat depends on a number of factors. The biggest determinant of sweating is humidity. If there is a lot of humidity in the air, then your sweat cannot evaporate quickly which causes you to sweat more. You can think of humidity working like a wet suit holding your body heat in. Also, the more you work out, the more efficient your body becomes at cooling itself down. This means that you sweat more when you’re in better shape. Most professional athletes have a very high sweat rate because they are highly trained. It is good to sweat but it’s also important to replace the water lost during exercise by rehydrating. If you’re working out in the summer months or in a hot climate, try weighing yourself before and after your workout. If you lose any weight during the workout, you must rehydrate until you replace the water lost.

There are many self-evaluation things that are “measurable”: your weight, % body fat, how much money you make, the value of your possessions….all of these and more are “measurable”. However, a number of self-evaluation things are not measurable: self-esteem and self-confidence are included in this category. Is there any doubt that your successful weight loss efforts enhance these? I am very fortunate to witness this daily in my practice. When my patients lose weight, you can almost feel the cloud lift from ow self-esteem and confidence in favor of a much higher level of both. The way people react to you, the way you see yourself in the mirror, etc. Your successful weight loss efforts will produce a incredible increase of a number of “unmeasurable” self-evaluation items that will make your life ultimately a happier one, aside from a healthier one.

Have you ever had a strong craving for junk food and it just wouldn’t go away? Believe it or not, the best way to fight a craving is to get moving. Exercise will get your heart pumping and will divert blood away from the digestive track. This is why we don’t feel hungry during a workout. Another amazing benefit from exercise is that it causes a release of endorphins (our feel good brain chemicals) which will help increase your mood and motivation making it easier to stick to your dietary plan. Many people have food cravings when they are depressed or stressed out. If you can develop an awareness of what triggers your cravings, you can alleviate your mental and emotional struggles by working out. Next time you are daydreaming about chocolate cake, try going for a jog. If your cravings strike late at night when it’s not feasible to go to the gym, try doing some bodyweight moves like crunches, push-ups, and squats.

Last night your Doctor was in D.C. for dinner at a very nice steak restaurant. I started thinking about the “extras” that really would turn this elegant meal into a caloric disaster. Starting with the perfunctory before dinner “cocktail”, to the warm basket of bread and butter brought to the table to the offer for a great potato side dish to the after dinner dessert menu…all of these “extras” add boat loads of calories to what could be the same, elegant, wonderful steak dinner. The “drink” before dinner could be a sparkling water, who really needs the bred? …an appetizer a great shrimp cocktail, crab or salad, a huge steak for dinner, broccoli as a side and is anyone truly “hungry” for dessert after consuming this large meal? Okay, now all of you are hanging on your seat waiting to hear what Posner did…okay, I confess: I had a light beer (still alcohol in any form is detrimental to weight loss, I had that warm bread, I had potato skins as a side, but I passed on dessert. I blew it…the “extras” got to me…and I couldn’t even finish that wonderful prime rib because I was full from the extras.

When do you think you make the “not the best” food choices: when you are alone or when you are with others? When alone, several issues occur: 1- there is anonymity in food or drink gorging…no one else sees your detrimental eating behavior. If you have a spouse who is encouraging your weight loss efforts, you will feel uncomfortable eating derailing food sources in front of that person for fear of being admonished or looking weak. Also, when you are with others, there is conversation, activity and other activities occurring that lessen boredom, and boredom leads to mindless eating. However, in the social company of others, the “herd” mentality takes over so if everyone os eating and drinking alcohol, you want to join the rest. Being alone allows you to not be swayed negatively by others. Interested in your thoughts…comments?

If you haven’t tried yoga yet, you probably should. There are many health benefits of becoming a yogi. Doing yoga three times a week helps banish and prevent body aches and lower back pain by strengthening and balancing out muscle groups. Also extremely important, you can improve the quality of your sleep by doing yoga poses before bed. Research shows that doing yoga on a regular basis will help you achieve deeper sleep. Some mental benefits of yoga include increasing body awareness, improving self esteem, and relieving stress. This great exercise tones every part of your body and especially strengthens core muscles making it a very effective way to strength train. Try taking a few different yoga classes to see which ones you like best, then incorporate yoga into your weekly routine.
